Payment must be received in full 48 hrs prior to the booked appointment of any services taken out with GPMedic and the Garden or Kings Oak Hospitals. All prices are 'Total'. This means that there are no additional costs. They include Doctor's fees (where applicable) and all investigations. This includes a full printed report sent within 10 working days of the appointment with the doctor. Methods of payment are: Bank e- transfer, money orders and cheques. Cheques must be made payable to the doctor and contain the necessary cheque guarantee card information on the reverse.
After your booking has been processed we will forward you the necessary documents which include a welcome letter and introduction pack , a medical and lifestyle questionnaire which will help us analyze your holistic report. This should be delivered within 2-3 days and then you will be required to return the completed questionnaire and the payment in full in the self addressed envelope provided.
You will also be sent the investigation request form stating the package chosen. You will need to come to the booked appointment 45 minutes prior to the arranged time to have the necessary investigations before seeing the doctor. If this is not convenient then a separate time can be arranged to have the investigations. This may however cause delays in the report. The total time required can take up to 2 hours. One full hour with the doctor.
ALL HEALTH SCREENING IS CARRIED OUT AT EITHER THE KINGS OAK HOSPITAL (Enfield) OR THE GARDEN HOSPITAL. (Hendon)
THE MEDICAL SCREENING WILL BE CONDUCTED BY FULLY REGISTERED GENERAL PRACTITIONERS. REGISTERED WITH THE GENERAL MEDICAL COUNCIL AND THE ROYAL COLLEGE OF GENERAL PRACTITIONERS. ALL THE DOCTORS ABIDE BY THE GMC CODE OF ‘ A GOOD DOCTORS’ CONDUCT.

The report will be sent to you but a photocopy of the report will be sent to your GP unless otherwise requested.
You will receive a call once we have received the completed forms and payment in full has been cleared. This will confirm that we have received your details and to confirm the appointment.
Once an appointment is booked and payment is received the cancellation policy is as follows:
Cancellation and/or rescheduling of the appointment with more than 5 days prior to the appointment will incur a 10% administration fee.
If this notice is given less than 5 working days prior to the appointment, then a charge of 25% of the total fee paid will be incurred to cover costs.
If 6 weeks notice is given then no charges will be incurred.
Your receipt will be enclosed with your report. You will also have the option for annual recall.
